The Division of Taxation has updated Forms GIT/REP-3, GIT/REP-4, and A-3128. The updated version (8-24) of the GIT/REP forms and A-3128 were released in August 2024. The previous version of the GIT/REP-3 (11/23) and GIT/REP-4 (8/21) are acceptable for deeds and signed GIT/REP forms dated prior to September 1, 2024. However, for deeds and signed GIT/REP forms dated on or after September 1, 2024, the current version (8-24) of the Forms GIT/REP-3 or GIT/REP-4 must be used. Only the current version (8-24) of Form A-3128 may be used for requests dated on or after September 1, 2024.
